More patient safety errors are associated with medication than any other factor in healthcare. These can occur at every step in the chain, from prescribing the wrong drug, through dispensing and storage, to patients taking the wrong dose or failing to complete a course of medication. Many risk factors are design-related, particularly with regard to packaging and information graphics. The Helen Hamlyn Centre has worked on these issues for several years, resulting in a series of design guidance publications, which are making an impact on industry and medication pack design.
